REQUIRED PAPERWORK FOR REGISTRATION

• Completed PWAA Membership form
• Current picture
• Copy of Birth Certificate or current Passport
• 3 copies of 2009-2010 final report card (for all but flag football players)
• Physical Form (signed by Physician in 2010)
• Participant Contract form
• Registration Fees, Snack stand deposit

REGISTRATION FEES

Per participant fees:
Flag Football - $75.00
Challenger League Flag Football - FREE
Tackle Football - $100.00 / ( $125.00 after April 6 )
Cheerleading - $100.00

Per family fees:
$50 snack stand deposit ( separate check )
$40 fundraising fees
$25 membership fee (unless paid during 2009 Penndel baseball/softball season)
